Reason:

Balls is ambiguous, does it refer to the testicles themselves? A beach ball or something other like multiple rubber balls?

It doesnt account for artwork involving castration fetish like exposed_testicles or etc

-1, because balls_(disambiguation) would do a better job of explaining beach balls and the sort. Plus, balls is already a universally-accepted term.
Even in the case of castration, testicles are still part of the term "balls". Perhaps implicating scrotum to balls would be a better idea.

That's fair, balls_(disambiguation) would work well for those tags, but my consideration comes from a couple things really.

One, cock and dick are universally accepted terms for penis, but cock and every tag using it has changed to penis.

And also tagging balls is something we do when we specifically see the scrotum. Its not really something we can reliably tag unless a scrotum is there and in cases where art has an internal anatomical depiction of a testicle it would be still tagged as balls, but thats a problem for people who'd want to not see that sort of thing.
