But that smiley clearly shoes a person licking their nose! (We have no way of distinguishing if the tag was :D or :d thanks to no case sensitivity)

That's why the Wiki has a clarification: "Not to be confused with the action of licking the nose. Tags use lowercase letters only and this tag is meant to represent the symbol combination :D, not the lowercase :d."

I always thought it to be the half-smirk/grin, where only one side of the mouth opens while smirking/grinning.
However, if you know the answer, why do we only use lowercase? I usually cap the letters in proper names, even when it's one word names with no spaces.
