I don't know of a tag to indicate "clustered nipples", more than one nipple on one breast or areola. Could maybe try to start a clustered_nipples tag, or something similarly named, but I don't know if it'll catch on and be used correctly.

I can make it a personal project of mine, but still it does seem like a difficult thing to name correctly.
Perhaps a "multi_nipple_per_breast" would do as an umbrella tag, it's a bit long for a tag, but it'll show up in the autocomplete for "multi_nipple"

It doesn't really have to be on breasts, though, does it? Could be non-breast udders, or flat-chested, or flat along the abdomen like teats.

...true, yeah.
I still don't like clustered_nipples because it feels very unlikely you'd stumble upon it when trying to get the correct tag.
Maybe some aliases/implications would help: (four_nipples_per_breast aliased to quad_nipple) implies clustered_nipples, (something like triple_nipples?) implies clustered_nipples.
Or we could go the (number)_breasts, *_fingers, *_toes route, and create *_nipple_cluster tags, but eeeeh
