No. For urethral_fingering and urethral_penetration, extreme size difference and/or hyper genitalia could make it not torturous at all. Also, testicle_cuff and cock_ring don't need to be worn to be tagged (they just need to be present in the image), and even if they are worn, the size of the character could negate any potential torture.
