I don't think this is a foursome. What if they were all the way across the room, in separate beds? They could still swap out at any time, but that doesn't make it a foursome. There is definitely a difference between "an image in which two people are at it" and "an image in which two pairs of two people each are at it". I can see why you don't want to call it a foursome, but it's still something that some people are interested in, so it has to be distinguished somehow.
