I had uploaded a picture, and now it's not listed under my recent uploads or in a search on the site. Was there some kind of server hiccup, or is there a way to tell if it was removed or denied?

photomorphs generally (as far as I have seen) don't get past the mods very easily. I think it's less to do with the mods themselves and more to do with it's REALLY REALLY easy to screw up a photomorph. Super easy - there is a thing called the uncanny valley and photomorphs can very easily fall into said valley, which makes them very bad for a (basically) porn sight.
Could you upload the image to "imgur.com" and give us the link here on the forum? I might be able to tell you why (if not someone else might)

There's also the problem of "This is real-life porn," even with the photomorph, meaning it won't belong here.
