Overview: We should have a box to enter an edit reason when using "Undo" or "Revert To" on the tag history page.
Usefulness: It would allow us to say why we're undoing or reverting edits.

Somewhat related: Can we get a search function for tag history to find specifically the black + or -? Would not be overly useful in a specific image's history, but very useful for a user's history. When trying to find out which of your own edits got undone through hundreds of images, it would be a vast help.
