I think the problem with double_fellatio should be obvious in that it covers two opposite things: two characters receiving fellatio from a third, or two character performing fellatio on a third.

We can either split the receiving to something like dual_fellatio (after dual_wielding), or we can split the giving case to something like collaborative_fellatio.

Given that the "giving" case is the rarest, I think it would be best to move those out of the tag myself. That means that the double_ parallel with double_handjob, but that can easily be aliased to dual_handjob anyway.

Well, it provided me with the idea for the other renaming option XD

I support moving the "performing" case to collaborative_fellatio, on the grounds that that's a very clear tag name. Dual_fellatio and double_fellatio having different meanings would be begging for mistagging.

When I saw this thread, I thought it would be about how double_fellatio is currently applying to both fellatio and penis_licking, even though their wiki pages separate them.

Using double_oral to cover more broadly is certainly an option, but I suspect such things get more common the more precisely you tag.

E.g. we have both panty_pull and pants_pull, which then requires us to have a separate underwear_pull, because panty_pull implies panties (although I see no reason why it is panty_pull and not the broader one in the first place). Similar, the fact we have skirt_lift leaves things like post #572733, post #592247 and post #571381 orphaned of a good tag.
