There's also creepy, which seems to be used for 'milder' but still unsettling content
Honestly I don't think there's any standard usage for any of those tags and similar, like:
what
why
what has science done
where is your god now
etc.
I've seen giant penises get tagged with what, as well as transformation and penectomies/castration, some gore (neck sex), surrealism, 'strange' art styles, odd anatomy etc.

there is soul devouring eyes too, but that seems specific enough

On a related note, most of the posts in forum #188064 would likely be considered nightmare fuel, along with the above tags, but for different reasons by different people. Not to mention some wouldn't consider them 'scary' at all

The difference between creepy and nightmare_fuel is so subjective that it might not even exist. What's merely spooky to someone might be outright nightmare fuel to someone else. And then there's that problem of hardcore kinks getting tagged as nightmare_fuel.
And posts such as these have both cute and nightmare_fuel tags:

...sometimes I think that it might be best to just alias creepy, scary and nightmare_fuel together, and then instead focus on tagging the features that might be considered 'nightmare_fuel'. Sharp_fangs, evil_grin, uncanny_valley, etc.

Why is that second one tagged as nightmare fuel? The only thing "creepy" about it is the eyes, which is covered by soul_devouring_eyes

Not sure why, but I suppose it might be borderline uncanny valley. There's something off with the face and the pose. Oh, and the wings seem to be fake.
Like I said, nightmare fuel is subjective.

There was a suggestion a while back about using sets/pools for tags like these, but don't think anything came of it
You could still do conjunctive searches using metags pool: or set:, but not everyone either knows that, or wants to do that

Those images are all of conjoined_twins. I've sent them a message about it, hopefully they'll start tagging that instead of what.
Edit: Actually I'm not sure about that second one. Normally you'd get two snouts if the eyes are separated, should that still be conjoined twins?
