Let me start this off by saying how much I hate this tag's subjectivity. We're going to fix this.

I feel like this can be a useful tag, but I'd like to discuss how we use it now versus how we should use it.

I've come up with these possible ways to define it:

• blank and/or intense stare (death_stare?), coupled with an intense emotion, such as anger
• distinguishable from rape_face?
• plus clenched_teeth, or an absentminded open_mouth
• hunched_over, or some other unusual posture
• presence of a corpse, a weapon and copious amounts of blood, a murder or imminent_death/imminent_rape
• some evidence of obsession, such as a background wall covered in posters or candid photos of another character. Stalker/being_watched
• another character is nervous in the insane character's presence
• may be wearing a straitjacket

Hopefully we can clean this up for consistent use in the future.

This actually feels like a tag I would want but I can imagine all the problems without looking at your list.

To be honest I don't see how we're going to be able to keep this tag "clean". I mean it feels like such an inherently subjective tag in many ways to the point I'd just say either keep it as is and throw some suggestions into the wiki or scrap it (which I personally don't want to do).

By the way, are there any similar tags around?

None that I can think of right now.

There's a tag for the mad_scientist stereotype, but it's not quite the same as insane.

The old crazy_eyes tag hasn't been used much, and some of it seems subjective.

...yeah, there's not a whole lot of tags for craziness.

There's also madness, but it's mostly used for other things and needs to be sorted out someday.

Those might fit under psycho/psychotic.

Would it be safe to alias those to insane?
