How can I fix already uploaded content? I created a new version of an image or video, how can I replace my old content with errors with a new one ?

In most cases, you can upload the newer version and flag the older one for deletion. This will count as one deletion penalty against you.
Alternatively, you can work with somebody who has access to Replacements. While approvers like myself can choose not to penalize you when your content is replaced, approving the replacement will move the post to the replacer's account.

I uploaded a new version of my video. Can you delete my old video ?

you can flag the post you wanna replace as "inferior or duplicate post" and then a mod/janitor will come over and delete it
