That's actually a good point, but also makes me ask what's considered visibly ill. The one situation I can think of is a character seeming fine but having a thermometer show their temperature to be in fever range.
If that were to count as visibly ill, then I can't think of any other situations. If it doesn't count as visibly ill then I'm also fine with self-rejecting the implication.

Yeah that was the situation I was considering. It's not visibly ill in the sense that the person looks fine, but you can visibly see that they have a fever from the thermometer.
