I feel that these tags are a bit too ambiguous. It's used if, say, cum gets into a certain orifice, or if it's coming OUT of that orifice. Maybe it should be repurposed to actually be for the act of coming inside of that orifice, like a creampie, or something of the sort.
Topic: "cum_in_x" tags should be repurposed.

I don't see how changing their definition to something less intuitive (cumming inside vs cum being inside) will help anything

Those tags are cum_from_[orifice]. Tends to be either of leaking_cum or cum_splatter (forceful ejection/backsplash).
the act of coming inside of that orifice
cum_in_[orifice] (cum_inside) + ejaculation. Can be hard to remember to tag ejaculation. [orifice]_penetration + cum_inside is probably great at finding untagged ejaculation.
