I think the alias should go the other way around.
There are a few examples in heart_gesture that have the heart formed by other appendages, such as a Sylveon's ribbons. It'd be better for the tag to be general enough to cover these, instead of specifying that the heart is formed with hands.

hand heart could also be misinterpreted as something like a heart marking on the hand.
