I would assume that the former is when the cum is somehow produced through lactation or some other internal bodily process, while the former is for situations where the cum is a foreign substance. but the wiki pages for both tags seem to say more or less identical things.

And if one implies the other, it would make it harder to find or blacklist posts where the cum is a foreign substance and leaking out, as it would also include self-production of cum that leaks out of nipples.

Most of the posts under lactating_cum currently seem to be a cum_through situation, so if this is meant to be the distinction, it doesn't seem like most people have been following it...
