A smily face does not mean eyes closed. This is a stupid implication

I'd imagine it'd be fairly rare to find this sort of exception... I would just tag it something like

canine breasts pregnant big_belly waving heterochromatic skirt green_skirt female ^_^ eyes_open

Or we could use ^_^_(symbol) for such an instance.
