Black and white is the specific dichotomy of black and white. Monochrome is every kind of simple monochromatic color scheme.

I don't think the tagging is entirely consistent. Some images have merely a single hue and so may have black, blue, grey, or, white. I'm not sure what tag these get but they deserve a tag by themselves.

Those are still technically monochrome. Monochrome, in the most literal interpretation, means simply "one colour".

I think he's talking about something that is black-and-X-and-white (and the shades in between), which is a technique that can produce striking results.
