The last two bubbles of the third panel are a bit out of order: you want to read the last bubble before the second to last one even though it should be the other way around. That could be fixed by swapping the bubbles’ positions horizontally so the “path” through the bubbles your eyes want to take doesn’t trip you up.

She obviously has a urethra inside her penis. Even ingoring all the times we've seen her ejaculate in other images, you can see the urinary meatus in the 3rd panel. "Down there" presumably means the perineum, the parts of her crotch that are hidden from view behind her dick.

The anatomically correct explanation would be that, since she's a snake, she'd have an all-in-one cloaca, serving as the singular orifice for urination, defecation, and copulation. But that's (thankfully imo) not how Kyra was designed.

A transgender woman who underwent the gender reassignment surgery would have a vageen. The breasts naturally develop during hormone replacement therapy (estrogen pills and testosterone blockers in tandem are most common), though it takes years and a gamble to get them that big, naturally.

She still would had been probably assigned 'male' at birth though, considering that she'd had been born with a penis but not breasts or any other dimorphic traits.

Them being 'natural' or 'hormone changed' (which is how 'natural' get there anyways) is a moot point.

That and many trans people (women or otherwise) don't get genital corrective surgery, and many don't want it.

The point stands though that the kid should have figured there was something up with at least one mother's anatomy, when he knew that he had two biological mothers.

He's naive but seems to understand at least what sex *is*.
