It's not actually 90°. Her vagina is facing the same direction as her butthole, you just can't see it for the sake of the "x-ray" shot.

That's actually the viewer being incapable of seeing the "folds" or "wrinkles" (Whatever you wanna call it) of her ass due to the angle.
