This tag should be used when a character sprouts a penis (and often balls) in some way, typically to the surprise of another character.

The first image features a pair of gargoyle who each have a featureless crotch, but after seeing an individual they find attractive in the second image, they each get aroused and suddenly sport genitals -- to the surprise of said individual -- in the last image.

This may also occur while the receiving character is performing cunnilingus on the giving character, unaware that their throat is about to be filled by a cock that doesn't currently exist (or is hidden).

This nearly always means gender_transformation, specifically ftm_transformation or fti_transformation.
