This again? I kinda want to alias it to strategically placed hole so that the uploader can see it turned into something else and retag it. I also don't want this to become a tag cleanup project.

You could invalidate it, a grouping of three letters can be interpreted as multiple different things which has become evident. People could just use the full length of that tags

Granted a tag clean up isn't what you're looking for so the invalidation would probably be the best opt

I think an alias of sph -> sph_(disambiguation) may be a good idea since the acronym sph could stand for multiple things, and appears to currently be in common usage for both small_penis_humiliation and strategically_placed_hole (at least, to some extent).

Additionally, a cleanup of the tag wouldn't be too difficult since there are only 4 posts with the sph tag, and future maintenance of the sph_(disambiguation) tag can be limited by linking the small_penis_humiliation and strategically_placed_hole tags within the wiki for sph_(disambiguation).
