Reason:
The aliases are because official media writes out the number when referring to incarnations of this character. topic #23559, an implication request for fourth_doctor -> 4th_doctor, would need to be rejected before this BUR could be approved.
The implications are because these are all different incarnations) of the same character, although they have different appearances and personalities.

I'm not sure if the_doctor should stay as it is or if it should be the_doctor_(doctor_who) instead. What do you guys think, is it too ambiguous?

To be honest, there's characters from other franchises who could be labeled as "the doctor" simply because they have no official name, although the only one that comes to mind is the EMH from Star Trek: Voyager. It's also possible someone might forget the "the" when tagging, accidentally turning the Time Lord into just a medical practitioner. Using the suffix would be best, in my opinion, perhaps throwing in a doctor_(doctor_who) –> the_doctor_(doctor_who) alias to help taggers via auto-complete.
