Wouldn't an implication to lactating_juice (which could then imply unusual_lactation) be better here? Or an alias for simplicity's sake

I’d rather an alias, I don’t think we need separate lactating tags for every single kind of juice. Lactating_juice + orange_juice should be perfectly fine for searching purposes.

I wonder if we ought to use "fruit_juice" in these kinds of tags rather than simply "juice" just to remove that slight amount of ambiguity.

What about vegetable juice? I think I'd rather keep both of those under the same tag.

I mean, most vegetables are fruits, but I guess for the sake of like carrots and kale it might make sense to combine them.

The terms are mutually exclusive, at least in a botanical sense. Not that most people know the difference anyway...
