Use two brackets instead of one for tag references. Braces can do searches, instead.

I assume the first is saying "cum!" and the other is they actually do it? Not sure why it wouldn't be orgasm_command and spontaneous_orgasm together, instead for the second case.

That would also cover the domination fetish thing where it's not spontaneous but from direct stimulation. Replace spontaneous orgasm with something like fingering_partner+vaginal_orgasm, or handjob+ejaculation or whatever method is used to achieve it.
