There's not always an explosion. It could be used to signify someone being pissed, with no visible michael bay-splosion.

Disagree; mushroom clouds don't always come from explosions. Mostly, yes, but not always.

I agree with this. Mushroom clouds are synonymous with controlled explosions. Therefore, obviously, every mushroom cloud is caused by some sort of explosion.

Wrong. Mushroom clouds only appear when an explosion is occurring. After that, they're just normal clouds.
