Look at panel 2, ear and shoulder isn't that close as in last panel.

It's the blood from his ear dripping onto his shoulder/neck (I think). The fabric is the collar of his clothing (not really sure if I could call it a shirt).

I get what it's doing. The first half of the last panel is a close up of Devon's ear, and the second half is showing the blood dripping on his shoulder
