If you mean the yellow box with the butterfly, it's actually an "armored" medical case. In this reality, Fluttershy was partially in charge of medical research/distribution, and you can find her med supplies in cases scattered throughout the wasteland. Velvet uses it as both a saddle-bag and as light armor.
