Hermaphrodites are still separated into male and female due to the status of the mammary glands on the character, and whether the rest of the character's anatomy overall resembles a male or female more. The character's self-identification status gender-wise is also taken into account, but only when it's more ambiguous with the above traits.

Just want to note that gender has no place on e621, it's impossible to tag that reliably, hence it is not tagged. If sex is ambiguous, you either tag as ambiguous (if applicable), or you ask an admin.
