Looks synthetic, yeah, but I'll bet it's rubberized fabric, metallic thread cloth, or some other polymer/metamaterial.
It could be an amorphous metal sheet, but I would honestly find it kind of odd to make underwear out of expensive smartmatter.
Clothes? Sure! There's plenty that smartmatter clothes could be used for! But a loincloth covers so little, and exists solely for modesty.
The colour and pattern are probably either just to match the chassis/plating of his cybernetics for fashion's sake, or have some technological function that can't be divined from visuals alone.
