I really don't see why this would be so painful... It's not like he's ripping anything apart.

It's just like docking but with a hand instead of a dick and any species that has a knot or large nubs/spines on their penis tend to have stretchier openings to their sheath. So I would think any pain would depend on how hard you pulled.

That said if the pink ring shown here is fold in the penis skin (lots of nerves) because the penis is not fully extended to its full length then it could pontentially be very painful pulling on it.
