Maddie already looks strikingly similar to Keith, even having a scar on her eye just like his. I think it was originally part of her design because she was supposed to be a funny one-off character who would be "Keith's Stalker" while he was on the island, but then Tom made her an actual character.

Also, Basitins are represented by the mask of order, so that might have something to do with it

Don't beat yourself up about it. I only found out about it when Mark did a playthrough of Orcs Must Die 2 with Tom.
