With erotic fanart, you don't need to take the characters' backstories into account (unless you want to)--the 'actors' are simply the age they appear to be. Shirou appears older than Michiru, but not so much as to be creepy.

Canonically, Michiru has a crush on Shirou, and I think he finds it somewhat creepy. Not because of the fact that he's 6000 years old--its not like he'll find someone his own age--but because she is so obviously a teenager.

Personally I ship Michiru and Nazuna, they seem to have much more sexual tension than M. And Shirou.

Michiru turns 18 in literally the second or third episode. It highlights it at the beginning of the ep
