Estim isn't only a matter of voltage, there is also frequency, pulse-width, and current that factor into it. Given I've used about 600v sinewave with low amperage, I can say it can be pretty safe (although hardcore) if done correctly with proper equipment. ;)
In saying that, never use high current sources. 10mA over the heart is enough to kill you, never use estim over the chest or above the waist please! >_<
