See the joke is that the name is "running gag", so they split it into 2 parts, and make her a bulimic runner.

Although, to be accurate, she really shouldn't be thin, as binging and purging doesn't effectively contribute to weight loss.
The majority of bulimics are at a fairly average weight for their age/height.
