That's because it's neither panties nor a strap- it's a vegas-inspired unitard. Like a skimpier one-piece swimsuit, and it is pulled to the side. That there is a very sexy crossdresser.

It's definitely ambiguous_gender: it could go either way, but we can't tell from the image.
