I think what's happening here is that this particular character is being shown as a third gen (organic, but still completely artificially assembled and programmed) synth. The order of the pics can seem a bit confusing, but they're not flaying him down, but rather building him from the bones on up.

I played the fallout in the time Iyou commented, and I get it now. HOWEVER!!! synths do NOT have wieners!!! This is NOT cannon!!!
