    It's not unheard of. Half Life's version is definitely silly, but there were a bunch of special forces folks the world over who slapped one grenade launcher or another on an MP5, mostly in the 90s/2000s. SF back then were obsessed with customized SMGs, much like how they're now equally obsessed with ultra-short rail-festooned carbines in obscure proprietary calibers.
    Malaysian SF (PASKAL) are the only folks who still do that, apparently specifically for ship boarding/breaching actions, usually with Less-Lethal launcher stuff loaded.

    As Anonomenclature said, the idea of real military using MP5 with UBGL is not unheard of, but it's certainly very rare. The only two models that were ever used were M203PI used by Malaysian special forces, and ISTEC ISL-200 series in limited use by UK special forces.

    As for media appearances other than Half-Life, MP5 with M203PI first appeared in 1999 movie End of Days starring Schwarzenegger. Unfortunately, there was absolutely no media appearances of ISTEC ISL-201 model, and these two videos by Jonathan Ferguson is the most public coverage it ever got, not counting rare articles such as this brochure from Jane's Infantry Weapons.

    By the way, why is it called "Hazardous Environment Vehicle"? Did "V" in EnVironment bothered people so much? I never understood this change in Black Mesa.
