If it's an explicit situation (obvious sex), it's rated as explicit.

I've always gone by the genitals thing, it wouldn't be the only time an image has lead me to confusion. I guess the fluid on the Latias' backside is telltale enough for it to remain explicit despite it not really seeming like much, but being clear as to what its supposed to be. I've seen other images that looked similar like this that ended up tagged as questionable.

I'd agree that if it doesn't show anything explicit, it should be questionable, since it's What you see, not what you know.
