The explicit rating is used whenever the character in question's genitalia are exposed in the picture; because it is only the character's breasts and not actually their genitalia means that the questionable rating should be used rather than the explicit rating.

explicit tags include any sexual body part. unless theres some breast feeding going on boobs are normally sexual

Also, rating:e requires a visible sex act, or exposure of the anus or sex organs (genitals, as in required for reproduction); while they can be sexy, breasts are not sex organs.
