When a character's balls are drawn with a dramatic or notable amount of difference in sag (when hanging neutrally), or notable difference in size between the two balls.

While exaggerated in artwork, the scrotum is asymmetric in reality, causing one testicle to hang lower than the other. It is not fully confirmed why, but most believe that this helps with temperature regulation and/or help prevent injuries in an impact.

Size asymmetry without health complications also exists, as genetics can cause one testicle to potentially develop faster and/or sooner than the other during puberty (the shape of the testicles would still be the same, and the size difference should only be noticeable when the scrotum is in its relaxed/sagging state.
