I really think hat_only/headwear_only should be counted as nude too. I don't know if there's any edge-case exceptions, but hats are always an accessory rather than an article of clothing imo.

motorcycle helmets, full helms, and other whole-head helmets definitely feel like they'd ought to be cover-y enough to count towards mostly_nude.

I'd... tend to agree, although some headwear like veils and balaclava I think still ought to count.

I never use the *_only tags on posts like this, simply because they could be wearing other clothing, but you just can't see it. I expect people searching headhear_only are, in fact, looking specifically for posts where a character is entirely nude but for a piece of headgear. Not stuff like your example, where you can't even tell one way or another.
