It's a horribly obvious and really poor example of photoshopping. It looks like the piss is coming out of a half-inch wide hole that would either be a freaky genetic mutation, intentionally cut [OWCHIES] or maybe he practices urethral penetration...

The least they could have dine is thin the stream out a bit, to make it look slightly normal and is in itself a VERY simple modification, but they couldn't even do that :P
