extreme penetration
A penetration which wouldn't seem possible even without bones and organs in the way. It often involves extreme body stretching/deformation and may make it difficult to recognize what is happening - many people will think to themselves "wtf" upon seeing it. An extreme penetration involves penetrative objects thicker than the torso/waist of the character being penetrated.
