This has been rejected a lot of times before, most recently topic #38680.
The outside tag is tagged from the viewpoint of the image. It would possible to see outside_sex from inside through a window, hole in the wall, or etc.

Wait, what? That's pretty counterintuitive. I assumed that "outside" without clarification would refer to where the main scene is located, not the observer.
I think most people would do the same.

well, in either case, you can have a background character(s) considered for outside_* tags without the post being considered outside.
I have often wondered whether or not it would be more intuitive to tag inside or outside based on the characters’ location rather than the viewpoint.
Or maybe have tags for both…?

You know, that raises an interesting question. Reminding me of a point somebody made years ago.
Imagine a scene whereby a character is browsing through a photo album while in their living room, and they come across a photo depicting an outdoor scene. Maybe a photo of them hiking in a forest or engaging in outside_sex (or any outdoor activity).
If we could easily see the rest of the living room, it would be easy to tag the scene as being inside.
But what if it was framed in a way that takes up most of the screen/image (i.e., a closeup of the photo)?
Alternatively, what if it was a distant shot of a photo/painting in the background (where you can still make out a scene of a character being outdoors)?
Would it be safer to not tag either inside or outside respectively for that matter?
