What even is that internal shot?

I believe it's a multi-headed penis cervically penetrating a multi-chambered uterus. It looks like she has at least four ovaries as well. More ovaries means more eggs?

I'm reminded that in medieval times it was believed that human women had a seven-chambered uterus; three chambers (corresponding to three personality types) on one side would gestate males, the three on the other side would gestate females and a center chamber would produce herms (who I suppose only have one personality type). This was tied into medical theories of gender, personality, the origin of the soul, numerology, and cosmological ideas. It was all fairly 'hysterical' to make a really badly obscure joke.

looks like two of his heads went into the middle head's womb and the left head feels left out

Yep, that's why the left right-side head isn't into it, she's not one of the two being penetrated.

They share the same vagina so they should share that pleasure although it's possible only one of them does. That would further explain why she's not into it. Also porn logic.

I remember a case of an actual, in-real-life two-headed woman. They were originally going to be twins, but their embryos fused together. Each head had control of one side of the body, but everything in the middle felt "fuzzy." Regarding sex and pleasure, I believe (though I may be misremembering) that it was a shared feeling, but only half so, like neither got full pleasure because they each only felt half or something.

Interesting. Fortunately it's likelier a hydra shares the same sensations since they are naturally multi headed.
