The black fly is one of the most basic and harmless types of enemies in The Binding of Issac. The only "harm" it can do to the player is if there are multiple in one room and they're near the player buzzing around with the noise possibly overpowering the music. It's a fly, nothing special with no need for a description on what those look like.
